Many of the approximately 60 to 65 million Americans who live in rural areas face healthcare challenges unfamiliar to most urban and suburban residents. Healthcare facilities are fewer. Distances are further. The nearest pharmacy may be many miles away. Just finding a doctor and getting to an appointment can be difficult.

The financial and logistical challenges facing rural healthcare continue to grow. Over the last eight years, according to a study from KFF, rural areas have seen more hospitals close than open. Patient volumes are declining. And 44% of rural hospitals have negative operating margins, versus just 35% of urban facilities.

But, as the KFF study notes, one-third of all community hospitals nationwide are in rural areas, and rural hospitals account for about one-third of total hospitals in every state. In July 2025, Congress passed H.R. 1, the One Big Beautiful Bill Act, which includes funding for the federal Rural Health Transformation Program (RHTP) — a $50 billion, five-year initiative that aims to provide rural healthcare facilities and systems with the resources they need to strengthen healthcare access, quality, and outcomes nationwide.

What Are RHTP’s Goals and Strategies for Rural Health?

The RHTP focuses on five key areas, with grant allocations scheduled to be announced by December 31, 2025. According to the Centers for Medicare & Medicaid Services, half of the program’s funds will be distributed equally among all states that submitted approved applications.

CMS notes that the remaining 50% of funds will be distributed “…based on a variety of factors including rural population, the proportion of rural health facilities in the State, the situation of certain hospitals in the State, and other factors to be specified by CMS…”

The RHTP seeks to:

  • Use evidence-based interventions to improve disease prevention, chronic disease management, behavioral health, and prenatal care.
  • Help rural providers work together to coordinate operations, technology, primary and specialty care, and emergency services.
  • Strengthen recruitment and retention of community health workers, pharmacists, and individuals trained to help patients navigate the healthcare system.
  • Promote innovative care models, incentivize Accountable Care Organizations to lower costs, and shift care to lower-cost settings.
  • Support technical innovation by supporting access to remote care, improving data sharing, strengthening cybersecurity, and investing in emerging technologies.

The CMS RHTP site includes a list of activities that are eligible for funding, a timeline, and complete instructions on how to apply for funding.
