Cloud-based Portal Enables Care Coordination within Accountable Care Organizations and Patient-centered Medical Homes

LAS VEGAS, HIMSS CONFERENCE, BOOTH #531—February 21, 2012—eClinicalWorks®, a market leader in ambulatory clinical systems, today announces eClinicalWorks Care Coordination Portal for Accountable Care Organizations (ACOs) and Patient-centered Medical Homes (PCMH). Tied directly into the electronic medical records (EMR) system, this tool will enable care coordination and reporting functionalities essential to these healthcare models.

eClinicalWorks Care Coordination Portal gives organizations the tools necessary to comply with regulations and optimize efficiency. Tying this functionality into the EMR, eClinicalWorks EMR or another, gives users greater ability to effectively provide enhanced care in this environment. Built on eClinicalWorks Electronic Health eXchange (eEHX) private health information exchange, this portal includes:

• Care plan documentation for care management;
• Patient engagement via text, voice and e-mail;
• Home-monitoring device interfaces;
• Transition of care plan management; and
• Integration with eClinicalWorks Community Analytics.

“Our customers are looking to eClinicalWorks to provide a solution for managing their ACO and PCMH initiatives,” said Girish Kumar Navani, CEO and co-founder of eClinicalWorks. “This solution will allow care coordination from a variety of environments, giving organizations the tools they need to effectively undertake comprehensive care delivery.”

ACOs and PCMHs have emerged as new models for delivering healthcare by coordinating care between patients and members of the healthcare team. The goal of this coordination is to help strengthen the physician-patient relationship, leading to better care and reduced avoidable costs over time.
