New Module Assists in Managing Population Health

Designed for communities and Accountable Care Organizations (ACOs), Community Analytics provides reporting, alerting and messaging capabilities necessary to manage population health.

Built into the electronic health record (EHR) system, public health experts are able to evaluate population health and quality improvement activities throughout the community. Reporting capabilities in this tool allow the creation and policy-based distribution of queries to obtain aggregate data. Alerting functionality provides clinical decision support alerts at the point of care for patients who meet specific conditions. Secure messaging allows HTML-formatted electronic communications to be sent directly to provider inboxes in the EHR.

Community Analytics enables reports to be run at the community level to determine and monitor adherence of clinical process and outcomes. The technology is flexible for evolving quality initiatives and public health needs. An example of Community Analytics in use is during a medication recall, communities are able to determine the number of affected patients, message providers and distribute an alert to the practices. Community Analytics also aids with chronic disease management, including diabetes.
