Medicare Accountable Care Organization Chooses eClinicalWorks Care Coordination Medical Record Solution to Advance Accountable Care Organization Goals

WESTBOROUGH, Mass.—February 10, 2014—eClinicalWorks®, a leader in ambulatory healthcare IT solutions, today announced that Georgia Physicians for Accountable Care (GPAC), LLC, has selected eClinicalWorks Care Coordination Medical Record® (CCMR) for advancing its physician-led Accountable Care Organization (ACO) objectives for its 36,000 members, including coordinating care across the continuum, evaluating population health and quality improvement activities. GPAC was named an ACO earlier this year by the Centers for Medicare & Medicaid Services (CMS).

“We have been at the forefront of improving the quality of patient care in Central Georgia for years and have achieved quantifiable results in that area,” said Dr. Shahriar Sedghi of Georgia Physicians for Accountable Care, LLC. “As a large ACO with multiple locations, we manage the care of our 36,000 members and selected eClinicalWorks because it offers the most comprehensive population health management tool available. We expect to use analytics, care planning and the referral network through eClinicalWorks CCMR.”

The ACO care delivery model coordinates care between patients and members of the healthcare team with the goal of helping strengthen the physician-patient relationship, leading to better care and reduced avoidable costs over time. GPAC participates in the Medicare Shared Savings Program, which has been created by CMS for facilitating coordination between providers to improve the quality of care for Medicare Fee-For-Service (FFS) beneficiaries, improve patient satisfaction and reduce unnecessary costs. eClinicalWorks CCMR will aid GPAC in meeting its quality measures and performance metrics.

“Georgia Physicians for Accountable Care is leading a care delivery organization that brings together independent physician groups to increase the quality of healthcare,” said Girish Navani, CEO and co-founder of eClinicalWorks. “eClinicalWorks CCMR delivers powerful analytics and insight into patient populations with closed-loop analytics making that information actionable at the point-of-care. We are delighted that our investments in population health management are getting fantastic traction with organizations, including Georgia Physicians for Accountable Care.”

eClinicalWorks CCMR is designed to provide visibility into a patient’s care across all settings, facilitate smooth transitions of care, engage patients and providers in preventative care, improve outcomes among populations and reduce costs. A vendor-neutral, open network securely connects ambulatory EHRs with hospital in-patient systems and payor claim feeds, including CMS, to provide cost and utilization for services delivered to patients.

Included in CCMR, eClinicalWorks Community Analytics™ uses closed-loop analytics to deliver reporting, alerting and messaging capabilities necessary to manage Population Health. Dashboards can be shown at the community level to determine and monitor adherence of clinical process and outcomes, for including medication recalls and chronic disease management. Care coordination and analytics work together for a collaborative care framework.
