Medicare Accountable Care Organization Chooses eClinicalWorks Care Coordination Medical Record Solution to Advance Accountable Care Organization Goals
WESTBOROUGH, Mass.—December 16, 2013—eClinicalWorks®, a leader in ambulatory healthcare IT solutions, today announced that the Louisiana Senior Care Coalition has selected the eClinicalWorks population health management solution, eClinicalWorks Care Coordination Medical Record™ (CCMR), to advance its physician-led Accountable Care Organization (ACO)objectives. With 25,000 members in the Baton Rouge area, this agreement will achieve goals that include coordinating care across the continuum, evaluating population health, and quality improvement activities.
“About 50,000 Baton Rouge residents lack coordination of care, timely access to specialists, support services, and support for disease prevention and management,” said Joe Eaglin, business development officer for Louisiana Senior Care Coalition (LSCC). “Our organization delivers an innovative and highly integrated care system to this population, and we selected eClinicalWorks because it offers the most comprehensive population health management tool available, an end-to-end solution for analytics, care planning, patient engagement, and referral network.”
With 45% of its target population experiencing major health concerns, LSCC formed the ACO to coordinate care between patients and members of the healthcare team in order to strengthen the physician-patient relationship for better care and reduced costs. LSCC participates in the Medicare Shared Savings Program, which was created by the Centers for Medicare & Medicaid Services (CMS) to facilitate coordination between providers for improved quality of care to Medicare fee-for-service beneficiaries and reduce unnecessary costs. eClinicalWorks CCMR will aid LSCC in achieving the quality measures and performance metrics set under ACO and Patient-Centered Medical Home guidelines.
“Louisiana Senior Care Coalition is leading a care delivery project that brings together independent physician groups with facilities, including hospitals, nursing homes and home health agencies, to increase the quality of healthcare,” said Girish Navani, CEO and co-founder of eClinicalWorks. “eClinicalWorks CCMR has received wonderful market response due to the complete nature if this solution, including its powerful analytics and insight into patient populations. Closed-loop analytics makes that information actionable for providers at the point-of-care. We are excited to be working with Louisiana Senior Care Coalition to aid the organization in meeting its ACO and PCMH goals.”
eClinicalWorks CCMR is designed to provide visibility into a patient’s care across all settings, facilitate smooth transitions of care, engage patients and providers in preventative care, improve outcomes among populations, and reduce costs. A vendor-neutral, open network securely connects ambulatory EHRs with hospital in-patient systems and payor claim feeds, including CMS, to provide cost and utilization for services delivered to patients.
Included in CCMR, eClinicalWorks Population Health Analytics™ uses closed-loop analytics to deliver reporting, alerting and messaging capabilities necessary to manage Population Health. Dashboards can be shown at the community level to determine and monitor adherence of clinical process and outcomes, including medication recalls and chronic disease management. Care coordination and analytics work together for a collaborative care framework.
