New Jersey’s Horizon Health Center Selects eClinicalWorks for Care Coordination

Joint Commission Certified Primary Care Medical Home Using eClinicalWorks Care Coordination Medical Records Solution to Assist with Accountable Care Organization Goals

WESTBOROUGH, Mass.—June 10, 2013—eClinicalWorks®, a leader in ambulatory healthcare IT solutions, today announced that Horizon Health Center, a Federally Qualified Health Center (FQHC) serving Jersey City, is expanding its relationship with the company to include eClinicalWorks Care Coordination Medical Record™ (CCMR) for advancing its Accountable Care Organization (ACO)-related objectives, coordinating care across the continuum as well as evaluating population health and quality improvement activities. An eClinicalWorks customer since 2008, Horizon Health was the first FQHC in New Jersey to implement electronic health records.

Horizon Health uses the eClinicalWorks EHR software, which is enhanced to meet the stringent reporting and billing requirements that apply to FQHCs, ensuring there is no interruption in the funding that supports its providers’ work. Since implementation, Horizon Health has received Primary Care Medical Home Certification by The Joint Commission and nearly 1/3 of its patients are actively communicating with their medical providers using the eClinicalWorks Patient Portal.
