New York’s Largest DSRIP Program, Advocate Community Providers, Selects eClinicalWorks
Organization Serving 437,000 Patients is Utilizing eClinicalWorks for Population Health and Delivery System Reform Incentive Payment (DSRIP) Program
WESTBOROUGH, Mass.—December 1, 2014—eClinicalWorks®, a leader in ambulatory healthcare IT solutions, today announced that Advocate Community Providers (ACP) f/k/a AW Medical Office in the City of New York has selected eClinicalWorks Care Coordination Medical Record® (CCMR) for population health management and the eClinicalWorks Electronic Health eXchange (eEHX®) interoperability platform. These technologies will assist ACP in addressing the objectives of the Delivery System Reform Incentive Payment (DSRIP) program for its 437,000 patients, along with achieving measurable, incremental clinical outcome results demonstrating the initiatives’ impact on improving healthcare delivery in the state.
According to the New York State Department of Health, the DSRIP Program will restructure the healthcare delivery system by reinvesting in the Medicaid program, with the primary goal of reducing avoidable hospital use by 25% during the next five years. Up to $6.42 billion dollars are allocated to this program with payouts based on achieving predefined results in system transformation, clinical management and population health. A similar program is in place for several states, including New Jersey.

eClinicalWorks CCMR will provide ACP with visibility into a patient’s care across all settings, facilitate smooth transitions of care, engage patients and providers in preventative care, improve outcomes among populations and reduce costs. An EHR vendor-neutral, open network securely connects ambulatory EHRs with hospital in-patient systems and payer claim feeds, including Medicaid, to provide cost and utilization for services delivered to patients. The eEHX health information exchange will connect a variety of systems, creating a community record that will provide physicians with access to patients’ vital health information and allow physicians to exchange data more effectively.
One requirement of New York’s DSRIP Program is that organizations are to be recognized as a Level 3 Patient-Centered Medical Home (PCMH). eClinicalWorks solutions are NCQA pre-validated and provide organizations with 16.25 auto-credit points towards their PCMH recognition. As a result of investments by the company in PCMH, 70 percent of Medical Homes using eClinicalWorks have achieved Level 3 recognition, the highest level possible.
