North Carolina Health Center Chooses eClinicalWorks Care Coordination Medical Record Solution to Advance Accountable Care Organization Goals

WESTBOROUGH, Mass.—February 12, 2013—eClinicalWorks®, a market leader in ambulatory clinical systems, today announces that North Carolina’s Southeastern Health (SeHealth) is expanding its relationship with the company to include eClinicalWorks Care Coordination Medical Record™ (CCMR) for advancing its Accountable Care Organization (ACO)-related objectives, coordinating care across the continuum as well as evaluating population health and quality improvement activities. SeHealth has been utilizing eClinicalWorks comprehensive electronic health records (EHR) solution across 30 locations since 2010.

This initiative will additionally assist SeHealth in becoming a National Committee for Quality Assurance (NCQA)-recognized Patient-Centered Medical Home (PCMH). Both ACO and PCMH delivery models coordinate care between patients and members of the healthcare team with the goal being to help strengthen the physician-patient relationship, leading to better care and reduced avoidable costs over time.

“At Southeastern Health, we embrace a collaborative model of care as the foundation for the delivery of patient care,” said Lynn Wieties, vice president of the Provider Network at Southeastern Health. “eClinicalWorks has played a big part in our success by unifying providers across multiple locations while also supplying the tools to improve the efficiency and effectiveness of care. We look forward to working with eClinicalWorks to continually improve upon our services and allow us to continue our ACO efforts and in becoming a PCMH.”

“Both the ACO and PCMH initiatives will reinforce the physician-patient relationship, while also promoting the collaboration of care throughout our organization and the community at large,” said Dr. Dennis Stuart, chair of network operating council at Southeastern Health. “As a comprehensive health care system, we strive to keep patients engaged in their care through various initiatives. Working with eClinicalWorks will provide the tools to enable greater care coordination for our patients.”

eClinicalWorks CCMR is designed to provide visibility into a patient’s care across all settings, facilitate smooth transitions of care, engage patients and providers in preventative care, improve outcomes among populations and reduce costs. A vendor-neutral, open network securely connects ambulatory EHRs with hospital in-patient systems and payor claim feeds, including Centers for Medicare & Medicaid Services (CMS).

Included in CCMR, eClinicalWorks Community Analytics™ uses closed-loop analytics to deliver reporting, alerting and messaging capabilities necessary to manage population health. Reports can be run at the community level to determine and monitor adherence of clinical process and outcomes, for including chronic disease management. Care coordination and analytics work together for a collaborative care framework.

“eClinicalWorks has been at the forefront with aiding its customers through ACO and PCMH programs and is the only vendor delivering closed-loop analytics for better care delivery,” said Girish Kumar Navani, CEO and co-founder of eClinicalWorks. “We look forward to working with SeHealth and its providers to reach the quality of care goals that they aim to achieve.”
