Medicare Accountable Care Organization Chooses eClinicalWorks Care Coordination Medical Record Solution to Advance Program Goals

ORLANDO, HIMSS Conference, Booth #2515—February 24, 2014—eClinicalWorks®, a leader in ambulatory healthcare IT solutions, today announced that Tampa Bay-area’s West Florida ACO has selected eClinicalWorks Care Coordination Medical Record® (CCMR) for advancing its physician-led Accountable Care Organization (ACO) objectives across 60 practices, including coordinating care across the continuum, evaluating population health and quality improvement activities.

“We formed West Florida ACO in order to improve patient care through better care coordination and achieve enhanced health outcomes,” said Dr. Jayadeva Chowdappa, chairman and CEO of West Florida ACO, LLC. “As a primary care-centric ACO connecting independent practices, we selected eClinicalWorks for population health management because of its strong reputation and ability to scale as we grow. eClinicalWorks will enable us to gain a 360 degree view of our patients’ records and deliver the triple aim of better care, better health at an affordable cost.”

The ACO care delivery model coordinates care between patients and members of the healthcare team with the goal of helping strengthen the physician-patient relationship. West Florida ACO participates in the Medicare Shared Savings Program, which has been created byCenters for Medicare & Medicaid Services (CMS) for facilitating coordination between providers to improve the quality of care for Medicare Fee-For-Service (FFS) beneficiaries, improve patient satisfaction and reduce unnecessary costs. eClinicalWorks CCMR will aid West Florida ACO in meeting its quality measures and performance metrics.

“West Florida ACO has taken on an ambitious program, bringing together independent physician groups to improve care delivery,” said Girish Navani, CEO and co-founder of eClinicalWorks. “eClinicalWorks CCMR delivers one of the most comprehensive population health management solutions in the market, and includes powerful analytics and care coordination on a single platform. We look forward to working with West Florida ACO to help them meet the program goals.”

eClinicalWorks CCMR is designed to provide visibility into a patient’s care across all settings, facilitate smooth transitions of care, engage patients and providers in preventative care, improve outcomes among populations and reduce costs. A vendor-neutral, open network securely connects ambulatory EHRs with hospital in-patient systems and payor claim feeds, including CMS, to provide cost and utilization for services delivered to patients.

Included in CCMR, eClinicalWorks Population Health Management Analytics uses closed-loop analytics to deliver reporting, alerting and messaging capabilities necessary to manage population health. Dashboards can be shown at the community level to determine and monitor adherence of clinical process and outcomes, including medication recalls and chronic disease management. Care coordination and analytics work together for a collaborative care framework.
