The organization will also be utilizing eClinicalWorks CCMR for population health management. This technology will assist MHHC in coordinating care across the continuum as well as evaluating population health and quality improvement activities. Patients will also be able to access their personal health records (PHR) through the use of the patient portal and healow smartphone application.
