Petaluma Health Center, a Federally Qualified Health Center, is the winner of the 2017 HIMSS Davies Community Health Award. PHC works to ensure access to high quality, prevention-focused health care to 35,000 patients each year in Sonoma County, California.
Florida Community Health Center (FCHC) has been awarded a 2014 HIMSS Community Health Organization Davies Award of Excellence. FCHC has demonstrated significant sustainable improvement of patient outcomes through the utilization of electronic health records (EHRs) and information technology while achieving notable return on investment.
Fremont Family Care, a small primary care practice in Fremont, NE is a 2014 HIMSS Community Health Organization Davies Award recipient. Using the EHR, the practice began tracking patients due for preventive care such as mammograms, colonoscopy and an annual physical. The rate of patients’ age 50-75 that had a colonoscopy rose from 37 percent in the first quarter of 2011 to 70 percent in the second quarter of 2014.
MedPeds was awarded a 2013 Ambulatory HIMSS Davies Award of Excellence, in the use of health information technology, specifically the use of the electronic health record (EHR) to successfully improve health care delivery processes and patient safety while achieving a demonstrated return on investment.
White River Family Practice (WRFP) was awarded a 2013 Ambulatory HIMSS Davies Award of Excellence. WRFP is a rural ambulatory care practice serving nearly 12,000 patients in Vermont and New Hampshire. Located in White River Junction, Vt., the practice has a team of six family physicians, three family nurse practitioners and an administrative support staff of fourteen.
Unity Health Care was named a 2012 HIMSS Davies Community Health Award recipient. As the largest CHC network in the District of Columbia, it serves nearly 95,000 underserved residents in 30 sites. According to the Davies information, Unity implemented a new phone system, piloted centralized scheduling, and implemented an EHR/PM system, contributing to a 21% increase in overall provider productivity.
